Payment is expected the day services are rendered. We accept Visa, MasterCard and Discover card. We can arrange financing of your treatment through one of two different companies. If you have insurance, your insurance will be filed electronically. You will pay any co-pay, deductible or non-covered item when the service is rendered. Should your account be unpaid-in-full after 60 days a finance charge will be assessed on your account in the amount of 1.5% of the outstanding balance per month (18% per year) It is essential that all changes in phone numbers and addresses be communicated to the office staff as soon as possible. Any change in insurance should be addressed prior to being treated. We will gladly submit your insurance. In the event that your insurance must be refilled because wrong data was given to our office or copies of x-rays or chartings must be made to substantiate the claim a $5.00 processing fee maybe charged to your account. Should you need a copy of your records, our office will be pleased to assist you. There will be a $10.00 fee for records duplication assigned to your account. We handle all emergencies as quickly as we can. Should one occur after hours, call the office phone number for the doctor's phone numbers. A treatment plan will be presented to you at the time of an exam or consultation. This is a working document, and it should be used as a guide to improving your health. Changes may occur in this plan as conditions improve or worsen. Our patients should review these plans and ask all questions of the doctor and staff until they are satisfied with the recommendations. We file your insurance as a courtesy. It is your responsibility to check on the status of your claim and to have your account paid in full within 60 days even if your insurance has not paid their portion. We call to get your insurance benefits. The benefits quoted are not guaranteed. The portion that you are asked to pay at the time of service is based on the information we have on your specific plan. We understand that different scheduling conflicts may come up. When you fail to make an appointment or cancel at the last minute it affects many other people. When rescheduling your appointment you need to call 48 hours ahead of your scheduled appointment time. A missed appointment fee of $25.00 may be assessed to your account for missed appointments or appointments cancelled in less than 24 hours prior to the appointment. Time and supplies are necessary for every appointment, and our patients are expected to up hold their part in keeping the schedule working for everyone.
