We strive to protect our patients' confidentiality at all times. We may ask you for some personally identifiable information, such as your social security number, birth date, address, and phone numbers. Much of this information is required for our records as health care practitioners, and is vital to our ability to serve you. As an example, your social security number may be required for us to submit your insurance claims accurately.

We always work to ensure that this information is safeguarded and only used when necessary. To do this, our office complies fully with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). One way that we work to safeguard our patients' privacy is to have each patient fill out a HIPAA form so that we know exactly who we are able to communicate your health information with (for example, spouse, other practitioners, insurance company). Another method we use to ensure your privacy is to submit our insurance claims electronically. While there are some circumstances where we are required to submit claims in paper form, most insurance claims leave our office through an encrypted electronic claims service.
